Case Managers needed for an organization that provides family shelter and supportive housing in Brooklyn, NY. This position is Monday-Friday 9a-5p. The Pay is 22.00-25.00 per hour depending on experience.
Their mission is to transform the lives of New York City homeless families by providing holistic solutions of safe housing, critical services, and ground-breaking programs they need to succeed on their own– so these families can regain their independence and their children can look forward to a brighter future.
Duties:
- Provide case management services to individuals and families in need
- Conduct assessments to determine client needs and develop individualized service plans
- Coordinate and monitor the delivery of services to clients
- Advocate for clients and connect them with appropriate resources and support systems
- Collaborate with other professionals, such as social workers, healthcare providers, and educators, to ensure comprehensive care for clients
- Maintain accurate and up-to-date case records and documentation
- Provide crisis intervention and support as needed
Required:
- Bachelor’s Degree required
- Must have minimum of 2 years of Case Management experience
